To complete the COMPASS profile, you will need to download the COMPASS Caregiver or Teacher form here. The Initial Consultation Guide can be downloaded here.

This form allows the Caregiver and Teacher forms to be compiled into a single form for easy comparison during the consultation

COMPASS-T for Transition Aged Students and Adults.

To complete the COMPASS Profile for Transition aged students and adults you can download the COMPASS-T forms here. For self-advocates, you can download the COMPASS Self-Report form here.

This form allows the Caregiver and Service Provider forms to be compiled into a single form for easy comparison during the consultation.

We have provided for you examples of transition resources from two states to download and print. These can be adapted for your particular state. We offer a Transition Guide for Indiana Residents which provides suggestions to help ensure a smooth and successful transition from primary schooling to post-high school years which you may download here. C-HOPE for Parents of Children with Challenging Behavior.

COMPASS for Hope (C-HOPE) is an 8 session intervention for caregiver’s of children with behavior. The overarching goal of C-HOPE is to promote stronger parent-child relationships by increasing parental competency, decreasing parental stress, and improving child behavior. C-HOPE follows a similar intervention method as COMPASS whereby parents or caregivers will attend one-on-one sessions with a therapist, and they will also attend group sessions with other caregivers who have children with autism to gain knowledge, find emotional support, and develop confidence in their parenting strategies alongside other families who share similar lived experiences. C-HOPE has been tested across several modalities and has been found to be effective at improving parental competency and stress while decreasing child challenging behavior.
